Manx Care has launched a new Health Passport to support more personalised and accessible care.
The document helps people share key information about their health, communication needs and what works best for them.
Developed by the Safeguarding Team for Health and informed by the 2025 Access to Good Healthcare report, the passport is designed for both children and adults, including people with learning disabilities, neurodivergent individuals and anyone who may benefit from clearer communication with healthcare staff.

It allows individuals, families and carers to record essential details such as communication preferences, medical history, sensory needs, behavioural cues and personal likes and dislikes, giving clinicians quick access to the information that matters most.
Safeguarding Practitioner Pete Corkill said the tool will help professionals better understand the person behind the diagnosis and could become transformative as staff grow familiar with its layout.
The passport is available as a printable document or an editable Word version, alongside an “About Me” sheet and guidance notes.
Manx Care is encouraging anyone who would benefit from a more personalised approach to complete one and share it with their care teams.
